In response to movement of the guide feature along the slot in the distal direction a first distance, a first end of the instrument may be advanced in the distal direction a second distance, which may be greater than the first distance. A device for one-handed insertion of a guidewire into a vessel or cavity is provided. The device has a body, an actuator carriage, a guidewire feed, and a syringe. The body has a handle, a guideway, and a clip to retain the syringe. The syringe has a barrel and a plunger with a flange and a guidewire passage. The actuator carriage has a trigger and a clip to grasp the syringe plunger flange. The actuator carriage slides along the body guideway. The guidewire feed has a wire feed surface. The thumb engages wire at the wire feed surface to advance and retract the wire through the guidewire passage and a finger moves the trigger to move the syringe plunger rearwardly and/or forwardly, while the remaining fingers of the one hand grasp the handle. The device allows the user to sense resistance encountered by the tip of the wire.

The guidewire torquer further comprises an insertion channel for receiving the guidewire. The insertion channel is arranged at a surface of the handling body, and the handling body comprises a shaping feature. The shaping feature provides tactile feedback as to the orientation of the guidewire, and the insertion channel provides a frictional fixation of the guidewire relative to the handling body.

Tortuosity of vessels at or leading to the site of minimally invasive procedures is a problem for conducting such procedures as they increase the difficulty to guide the procedural catheters through tortuous vessels. Methods and apparatus for stabilization of the sheaths and catheters during access, procedures, and withdrawal in these tortuous vessels are disclosed. The apparatus and method for improving access include application of a pull component to the stabilized procedural catheter using a stabilization catheter/wire in addition to a push component from the percutaneous access to make the access easier while enabling use of more flexible catheters and softer wires. These methods and devices address the problems of trauma to the vessels during access, procedure and removal of catheters and wires, improve pushability of softer catheters and wires, and also substantially reduce substantially the procedure time.
