A neurovascular catheter extension segment is provided, such as for distal neurovascular access or aspiration. The neurovascular catheter extension segment includes 1) an elongate flexible control wire having a proximal end and a distal end and 2) a tubular extension segment having a side wall defining a central lumen carried by the distal end of the control wire. The side wall of the tubular extension segment includes a tubular inner liner, a tie layer separated from the lumen by the inner liner, a helical coil surrounding the tie layer, and an outer jacket surrounding the helical coil. The extension segment may be introduced into the proximal end of a neurovascular catheter and advanced distally to extend beyond the catheter and thereby extend the reach of the catheter.

An occlusion balloon is inflated, causing carotid inflow to be diverted through the sheath and through a flow reversal region positioned in the jugular vein. After reversal of blood flow, a carotid intervention is performed.

The access sheath includes a hub, a cartridge carried by the hub, the cartridge having a first valve, a second valve spaced from the first valve along a central axis, and a spacer disposed between the first valve and the second valve, wherein the first valve and the second valve each have at least two slits that extend along, and twist about, the central axis, and a shaft assembly having a shaft hub and a shaft that extends from the shaft hub in the distal direction. The access sheath provides a liquid-tight seal when receiving the introducer and other devices to prevent leakage and blood loss and to decrease device insertion forces.

A valve with a cylindrical walled structure and a disc with a plurality of slits and flaps are located in the interior cavity of the catheter hub in which the cylindrical walled structure defines an open proximal end. A plurality of flexible ribs can be included and connect to an interior wall surface of the cylindrical walled structure and the disc with an actuator disposed inside the interior cavity of the catheter hub and having a distal end sized and shaped to project into the open proximal end of the valve.
