The present invention provides a brachytherapy applicator device having: an elongate body defining a plurality of brachytherapy needle channels, each adapted and configured to receive a brachytherapy needle; a proximal end defining a plurality of proximal openings, each in communication with one of the plurality of brachytherapy needle channels; and a distal end defining a plurality of distal openings, each in communication with one of the plurality of brachytherapy needle channels, wherein at least some of the plurality of brachytherapy needle channels are curved away from a central axis of the elongate body at the distal end such that a brachytherapy needle inserted therethrough will engage with tissue lateral to a cross-sectional profile of the distal end of the elongate body.

The system comprises a unit (10) configured to generate a constraint function representing clinical objectives relating to the treatment quantity, a selection unit (11) configured to determine, for each of at least some non-selected candidate device positions, a sum of negative derivatives of the constraint function with respect to the treatment parameter associated with the respective device position and to select a device position for use in the treatment based on a comparison of the determined sums, and an optimization unit (12; 408) configured to determine at least one optimized treatment parameter for the selected device position based on the constraint function.

Each of the shield assemblies has radiation shielding material extending about a circumferential portion of the tubular body and disposed between the outer surface and the cavity. An interlocking system is operatively mounted to the rotating assembly, and engages a group of the plurality of shield assemblies. The interlocking system is configured for transmitting a rotational input received from a driving mechanism to the group of shield assemblies, for synchronously rotating each shield assembly of the group about their respective longitudinal axis.

Systems, methods, and devices are provided for assisting or performing guided interventional procedures using. The system uses pre-procedure scans of a patient's anatomy to identify targets and critical structures. A measurement device containing position indicating elements or fiducials is placed in a pre-existing or physician created conduit or lumen whose geometry is known from a scan. During a procedure, the pre-procedure scans may be registered to the patient using the position indicating elements or fiducials and the geometry of the conduit. This registration may be used in an intervention to guide instruments that can obtain diagnostic information or provide therapy to the identified targets. During the procedure, the position indicating elements may be used to dynamically compensate for motion to further improve accuracy.
