There is proposed a bone fastener assembly comprising a bone fastener comprising a bone fastener head and a bone fastener shaft, a correction instrument and/or a locking insert, and a rod receiving head. The rod receiving head includes at least two rod receiving passages, and which can thus be connected to two rods. Moreover, the movement of the rod receiving head in relation to the bone fastener can be blocked prior to placing a rod into the rod receiving head. The proposed bone fastener assembly may be used for example for correcting larger spinal deformities.

In some implementations, two adjacent bone anchors may be advanced into respective, adjacent vertebral bodies of a spinal column. Cancellous bone may be compacted within respective inner chambers of the bone anchors. The inner chambers may comprise at least one of a plurality of bone engaging protrusions and a profile that increases in cross-sectional area, at least in part, from a proximal end of the inner chambers to a distal end of the inner chambers. A tether may then be coupled between the first and second bone anchors to apply a corrective force to at least a portion of the spinal column.

The shank head is uploaded through the lower opening to expand the retainer and the positioner contained within the capture region, after which the retainer moves downward into the locking region to capture the shank head. The insert is downwardly displaceable with tooling until a top surface of the insert becomes engaged with the horizontal ridge to inhibit upward movement.

The systems include plates having armatures configured to receive bushings, the bushings configured to pivot and rotate and thereby permit a greater degree of orientation of corresponding screws placed through the bushings. In embodiments, the bushings comprise anti-rotation elements which lock the bushings in a desired orientation. Methods for use of the components described herein are also disclosed.

A first cutting tool is automatically transitioned toward disengagement from the blank when 1) a relative axial position between the first cutting tool and the blank coincides with a first coordinate of a predetermined location, and 2) a relative rotational position between the first cutting tool and the blank coincides with a second coordinate of the predetermined location. The first coordinate is along the axis, the second coordinate is an angular position about the axis, and the first and second coordinates are defined by the coordinate system. A second cutting tool is moved into engagement with the exterior of the blank so that the second cutting tool engages the blank substantially at the predetermined location, in a manner enabling continuation of the thread.
