Restless Leg Syndrome (RLS) or Periodic Limb Movement Disorder (PLMD) can be treated using high frequency (HF) electrostimulation. This can include selecting or receiving a subject presenting with RLS or PLMD. At least one electrostimulation electrode can be located at a location associated with at least one of, or at least one branch of, a sural nerve, a peroneal nerve, or a femoral nerve. HF electrostimulation can be delivered to the subject, which can include delivering subsensory, subthreshold, AC electrostimulation at a frequency that exceeds 500 Hz and is less than 15,000 Hz to the location to help reduce or alleviate the one or more symptoms associated with RLS or PLMD. A charge-balanced controlled-current HF electrostimulation waveform can be used.

Closed loop system embodiments can automatically be reconfigured into a more suitable closed loop control system in response to measures of control system performance. Measures can be internal performance characteristics of the adaptive control system or external inputs provided by another subsystem. As these measures change in time, the robust adaptive system changes in response.

Embodiments of the disclosure include systems and method for spinal cord stimulation. A spinal cord stimulator may comprise a pulse generator comprising electronic circuitry configured to generate output current; at least one lead in communication with the generator and configured to extend into the epidural space of a patient's spinal column; at least one electrode contact located proximate to a distal end of the at least one lead and configured to provide electric stimulation to a portion of a patient's spinal cord; and at least one sensor located along the at least one lead configured to determine a distance between the at least one lead and a surface of the patient's spinal cord, wherein the generator receives the determined distance, and wherein the generator is configured to adjust the stimulation provided by the at least one electrode contact based on the determined distance.

There is provided a stimulation device for treatment of gait impairment of a patient. The stimulation device is configured to apply respective stimulation signals to electrodes bilaterally implanted in two subcortical regions of the left and right hemispheres of the brain of the patient, the subcortical regions being associated with motor control. The stimulation device is configured to apply respective stimulation signals having a rate of electrical energy delivered that is modulated with alternating waveforms at a gait frequency and out of phase with each other.

Automated Selection of Electrodes and Stimulation Parameters in a Deep Brain Stimulation System Using Sensed Neural Potentials

A method for optimizing stimulation for a patient having a stimulator device such as a Deep Brain Stimulation (DBS) device is disclosed. Test stimulation is provided at initial combinations of lead positions and values of a stimulation parameter such as amplitude, neural potentials are measured for each combination, and a neural response score is determined using the measured neural potentials. A next combination of position and a value of the stimulation parameter to test is determined using the neural response scores. This process repeats iteratively until a stopping criterium is met. The neural response scores and then used determine optimal therapeutic stimulation for the patient. Neural response measurements can also be used to exclude certain positions or stimulation values during subsequent optimization testing.

Neurostimulation in a neural fulcrum zone for the treatment of chronic cardiac dysfunction

Systems and methods are provided for delivering neurostimulation therapies to patients for treating chronic heart failure. A neural fulcrum zone is identified and ongoing neurostimulation therapy is delivered within the neural fulcrum zone. This neural fulcrum zone corresponds to a combination of stimulation parameters at which autonomic engagement is achieved, while the tachycardia-inducing stimulation effects are offset by the bradycardia-inducing effects, thereby minimizing side effects such as significant heart rate changes while providing a therapeutic level of stimulation.

Selective electrical modulation of neural populations

Methods of providing electrical neural modulation to a patient's brain are disclosed herein. The methods involve differentially modulating two or more target regions of the brain. For example, a first target region may be provided with an electrical neural modulation signal that activates that target region while a second target region is provided with an electrical neural modulation signal that suppresses or deactivates that target region. As the implantable pulse generators (IPGs) described herein include independent current sources, such differential modulation can be provided with a single IPG.

In some examples, a medical device is configured to deliver high dose electrical stimulation therapy to a patient by at least generating and delivering an electrical stimulation signal having a relatively high duty cycle, and a stimulation intensity less than a perception or paresthesia threshold intensity level for the patient. The pulses of the electrical stimulation signal may each have a relatively low amplitude, but due at least in part to a relatively high number of pulses per unit of time, a dose of the electrical stimulation may be high enough to elicit a therapeutic response from the patient.
