Medical systems, devices and methods are provided for engaging tissue, e.g. for clipping tissue, closing a perforation or performing hemostasis. Generally, the medical system including a housing, first and second jaws rotatable relative to the housing, a driver, and an elongate drive wire. The elongate drive wire may be disconnected from the driver, first and second jaws, and the housing, which are left in vivo engaged with the tissue.

An end effector for a surgical clip applier includes a housing, jaws that include opposed first and second jaw members, each comprising an independent structure movable relative to the other, the first jaw member defining a first inner surface and the second jaw member defining a second inner surface opposite the first inner surface, and an actuation mechanism arranged within the housing and operable to move the jaws between open and closed positions. The actuation mechanism includes an actuation plate longitudinally movable within the housing, and a transition pin extending from the actuation plate and received within corresponding slots defined in each jaw member. Linear movement of the actuation plate drives the transition pin through the corresponding slots and thereby moves the jaws between the open and closed positions.
Surgical instruments including devices for sensing tissue properties and methods thereof
A sensing device for sensing one or more tissue properties includes an adapter assembly, an actuation assembly, a shuttle, and a piston assembly. The adapter assembly is configured to couple to surgical handheld devices. The actuation assembly extends distally from the adapter assembly and is configured to operably couple to and be engaged by handheld devices coupled thereto. The actuation assembly includes a first drive shaft and a second drive shaft. The shuttle has a clamp and a shuttle sensor, and is coupled to the first drive shaft via a coupling. The shuttle sensor is disposed on the clamp. The piston assembly is coupled to the second drive shaft and configured to compress target tissue between the piston assembly and the clamp of the shuttle.
MEDICAL MANIPULATOR
A medical manipulator includes a flexible shaft, a distal-end portion, an operating unit with which the distal-end portion is operated, a wire transmitting a motive force of the unit to the distal-end portion, a coil formed from a material having a greater rigidity than the shaft and one end thereof being secured to the distal-end portion and the other end thereof being secured to the unit, and a tube having a greater bending rigidity than the coil. The unit includes a socket connected to the shaft, a ball relatively rotatable to the socket, and a handle with which the wire is pushed/pulled in the longitudinal direction, the coil is disposed at a position at which the coil covers an area surrounding the wire, and the tube is disposed at a position at which the tube covers an area surrounding the coil in spaces in the socket and the ball.
JAW APPARATUS AND TECHNIQUES TO MAINTAIN ENGAGEMENT UPON TISSUE SEPARATION
Disclosed embodiments include apparatuses for and methods of separating tissue with opposing jaw members having distal ends that translate toward a surface of the tissue as the jaw members are rotated outwardly to maintain engagement with the tissue. In an illustrative embodiment, an apparatus includes jaw members rotatably coupled adjacent to their proximal ends. The jaw members define opposing outwardly-facing curved channels disposed between their proximal and distal ends that slidably engage a distal pin. A bracket defines a distal socket that receives the distal pin. As the jaw members are motivated in a distal direction relative to the bracket toward the distal pin, engagement of the distal pin with the outwardly-facing curved channels causes the jaw members to rotate to cause the distal ends of the jaw members move outwardly transversely to the distal direction while the distal ends translate in the distal direction.

A treatment tool includes: a sheath; a treatment portion that is capable of treating a living tissue and is provided at a distal end of the sheath; a first driver and a second driver that are inserted in the sheath and are capable of driving the treatment portion; and a holder that is inserted in the sheath, and holds the first driver and the second driver. The holder includes a groove in which the first and second driver are inserted, and an intervening portion provided in the groove and arranged between the first driver and the second driver in a cross section perpendicular to a longitudinal direction of the groove that extends from a distal end to a proximal end of the holder.

Various embodiments of tool assemblies are provided having at least one rotary input coupling and at least one linear input coupling for allowing either a rotary output or a linear output (e.g., from a tool driver on a surgical robot) to activate at least one mechanism of the tool assembly. For example, mechanisms of the tool assembly can include a clamping assembly, a firing assembly, an articulation assembly, and a roll assembly. The clamping assembly can open and close jaws of an end effector, the firing assembly can translate a knife assembly through the end effector to fire staples and cut tissue, the articulation assembly can articulate the end effector, and the roll assembly can rotate the elongate shaft and/or the end effector.
SYSTEMS AND METHODS TO ENABLE PYLORIC CLOSURE
Devices, systems, and methods described herein relate to affecting an internal diameter of a body lumen, and, in many examples, of a pylorus. A silk-based bulking agent may be injected in a pyloric tissue so as to reduce an effective inner diameter of the pylorus. A multi-part occluding agent may be injected into a pylorus on the surface of the pyloric tissue to occlude the pylorus alone or in combination with the silk-based bulking agent.
