An electrosurgical apparatus and method for performing thermal treatment in the gastrointestinal tract, e.g. to ablate duodenal mucosal tissue. The apparatus comprises an instrument having a flexible cable and an applicator suitable for use with a gastroscope, which can be deployed within a patient to delivery energy in a targeted or otherwise controllable manner. The applicator can deliver microwave energy by radiation. The direct and depth-limited nature of microwave energy can be make it more effective than treatments that rely on thermal conduction. The applicator may include a radially extendable portion arranged to move an microwave energy delivery structure into contact with duodenal mucosal tissue at the treatment region. The applicator may comprise any of a balloon, bipolar radiator, movable paddle, and rotatable roller element.

Systems and methods for tumor ablation with controlled precision of a temperature profile utilizing a tumor ablation probe device may include disposing a distal end of the tumor ablation probe device in a tissue, the distal end including a plurality of electrothermal modules (ETMs) on probe arm(s), each ETM including a first surface component electrically connected to a second surface component; supplying a first voltage of a first polarity or a second voltage of a second polarity to at least one ETM, and repeatedly alternating between the first polarity and the second polarity based on a time sequence cycle. When the first polarity is supplied, the ETM heats the first surface component and cools the second surface component, and when the second polarity is supplied, the ETM cools the first surface component and heats the second surface component. Each ETM and/or probe arm is configured for independent control.

Another even more compact ESD is an integral one-piece portable device having a type of pistol hand-held grip, dis-connectable probe, and a rechargeable, removable battery in the handle provides approximately 30, one-minute treatments on a single battery charge.

Pulsed field ablation systems are disclosed, some of which may include an output pulse generation circuit and a high voltage pulse generation circuit electrically connected to the output pulse generation circuit. The output pulse generation circuit may be configured to generate an output pulse set at least in response to the high voltage pulse set. Each pulse in the output pulse set may have a rise time that is shorter than a rise time of at least one high voltage pulse in the high voltage pulse set.
