A medical guidewire assembly is movable through an exit portal of a guidewire introducer. The guidewire introducer and the medical guidewire assembly are each insertable, at least in part, into a patient. The medical guidewire assembly is configured to reduce, at least in part, the hoop stress surrounding a puncture hole extending through the tissue of the patient. This is done in response to movement, at least in part, of the medical guidewire assembly relative to the puncture hole (after the puncture hole has been initially formed).
Electrosurgical device for cutting tissue
A tool assembly for use with an electrosurgical device for cutting tissue includes a base portion, a return lead, an electrical insulator, a center pin, and an active lead. The center pin extends from the base portion and through a lumen of the electrical insulator. The active lead is securely fixed to the base portion and extends between the base portion and a distal portion of the center pin such that a portion of the active lead extends around the distal portion of the center pin and first and second segments of the active lead are spaced apart from the return lead. Upon activation, electrosurgical energy is transmitted from the active lead through tissue to the return lead to cut tissue in contact with the active lead.

METHODS AND DEVICES FOR VALVE CLIP EXCISION
A system for excising an implanted clip approximating opposed valve leaflets in a heart valve includes a capture catheter configured to be introduced proximate the valve leaflets on one side of the clip, a transfer catheter configured to be introduced proximate the valve leaflets on another side of the clip, and a cutting tool configured to be deployed between the capture and transfer catheters and to be engaged against tissue of at least one of the valve leaflets and to excise the clip. A removal catheter may optionally be used to remove the clip from the heart.

SURGICAL PERFORATION BETWEEN THE AORTA AND LEFT ATRIUM
Apparatuses and methods are disclosed for the perforation of a communication between the aorta and left atrium. The method includes introducing the apparatus, positioning the apparatus at a location along the aorta, and energizing the apparatus to create a perforation. For example, one method may include: introducing a flexible wire into the left atrium, advancing a dilator along the flexible wire to position the flexible wire adjacent a selected location along the aorta and energizing the flexible wire to create a perforation from the left atrium into the aorta.

Methods, apparatuses, and systems are described for stent delivery and positioning for transluminal application. The system may include a stent that is disposed coaxially onto an inner tubular member. In some cases, the system may include an outer sheath disposed coaxially along at least a portion of the inner tubular member. The system may include a distal cutting element coupled with a distal end of the inner tubular member and an anchoring component disposed at a distal portion of the inner tubular member. The anchoring component may be configured to retain a distal portion of the stent in place along the inner tubular member as the outer sheath is retracted proximally to deploy the stent, wherein upon retraction of the outer sheath, the stent releases from the anchoring component and expands into a deployed configuration within the body lumen.

An electrode assembly for an electrosurgical instrument includes a housing configured to operably receive a distal end of an electrosurgical instrument shaft, the housing encapsulating an insulative core sandwiched between a pair of return electrodes. The insulative core includes a slot defined about a periphery thereof configured to partially receive an active electrode. The active electrode and the pair of return electrodes are adapted to connect to opposite polarities of an electrosurgical generator. A tensioning mechanism is configured to tension the active electrode about the insulative core during assembly.
HEMOSTASIS METHODS AND APPARATUSES
A probe is configured with a flushing port and an evacuation port to establish a flow path to remove blood from a resected tissue. The probe comprises a balloon configured to expand and contact the resected tissue to compress filaments and improve access to the underlying blood vessels for coagulation with an energy source. An endoscope can be used to view the tissue, and the balloon may comprise a transparent material or a viewing port to allow imaging of the bleeding tissue through the balloon. The probe may have a light source to illuminate the tissue with a beam oriented at an oblique angle to the tissue surface, which can decrease interference from blood and may allow more localized coagulation of the blood vessel.
