Device and Method for Reducing Foreign Body Response from Neural Implants

A device for reducing foreign body response in a subject caused by an electrode implanted in a subject's tissue. A base is secured to the subject, having a base aperture in proximity to the target site. The base can receive and align a body thereon. A body contains a chamber extending between a chamber aperture, aligned with the base aperture, at one end and a chamber opening at an opposite end. The chamber contains an acoustic coupling medium, such as polyvinyl alcohol cryogel, transmits acoustic vibrations from a transducer without altering their frequency. The transducer is mounted to the device and is configured to transmit acoustic vibrations into the chamber and through said acoustic coupling medium to the subject tissue at the target site, creating an acoustic field in the target site sufficient to reduce foreign body response in the subject where the electrode contacts the target tissue.

In one example, a system is configured to deliver, via a first electrode combination, a first stimulation pulse to a portion of a spinal cord of a patient located caudally of a spinal cord injury location of the spinal cord of the patient; sense, via a second electrode combination, an evoked compound action potential (ECAP) signal elicited by the first stimulation pulse; identify, by processing circuitry, a characteristic of the ECAP signal; determine, by the processing circuitry and based on the characteristic of the ECAP signal, a therapy parameter value that at least partially defines a second stimulation pulse; and deliver the second stimulation pulse according to the determined therapy parameter value.

A mobility augmentation system assists a user's movement by determining a corresponding electrical stimulation for the movement. A wearable stimulation array includes sensors, electrodes, an electrode multiplexer, and a controller that executes the mobility augmentation system. The sensors measure movement data, and the mobility augmentation system applies a movement model to the measured movement data. The model can determine different electrical actuation instructions depending on the movement stimulated. For example, to stimulate a knee flexion, the movement model output enables a first set of the electrodes to operate as cathodes and a second set of electrodes to operate as anodes. To stimulate a knee extension, the first set of electrodes can be enabled to operate as anodes and a third set of electrodes as cathodes. The user can provide feedback of the applied stimulation, which the system can use to retrain the model and optimize the stimulation to the user.

Flexible sheet for neuromuscular stimulation

A flexible sheet for neurostimulation has a flexible non-conductive substrate matrix in which electrodes are embedded along a lower surface. Electrically conductive wires extend from the electrodes through the flexible substrate to another exterior surface of the substrate. Therapy field models may be useful in guiding the modification of therapy parameters. As one example, a processor compares an algorithmic model of a therapy field to a reference therapy field and adjusts at least one therapy parameter based on the comparison. As another example, a processor adjusts at least one therapy parameter to increase an operating efficiency of the therapy system while substantially maintaining the modeled therapy field.
