Radiation treatment planning and delivery for moving targets in the heart

Methods and systems are disclosed for radiating a moving object. The method may comprise acquiring a plurality of indicators of the phase of a physiological cycle of a patient and a plurality of images of the patient that include a target. Each image may be taken at a different phase of the physiological cycle and may be registered to the phase at which the image was taken. The method may also include identifying the target in each of the plurality of images, calculating a dose of radiation required to treat the target, calculating the number, orientation, and dwell time of one or more radiation beams required to deliver the calculated required dose of radiation to the target, and calculating a position of each of the one or more radiation beams required to achieve the calculated orientation. Each position may be a function of the phase of the physiological cycle to which each of the plurality of images is registered.

The system and methods may also include a heartbeat sensor that provides an independent reference indication of the cardiac phase to provide real-time or near real-time quality assurance of a current predicted phase indication. The disclosed system and methods may be configured for use in one of two modes: “beam-gating” and “beam-tracking”. For beam-gating, the predicted cardiac phase is compared to the desired gating window, based on the patient-specific treatment plan, to determine if a gate ON or gate OFF signal should be set. For beam-tracking, the predicted cardiac phase is used to load the appropriate beam parameters based on the patient-specific and motion phase-dependent treatment plans.

A system for adjusting radiation target sites dynamically according to the moving states of a target object and for creating a lookup table of the moving states includes a detection chip, a radiation generation device, and a lookup table. The detection chip can be fixed on the target object to detect the current moving state of the target object. The detection chip or the radiation generation device, both configured for wireless signal transmission to each other, can activate or deactivate the radiation emitters of the radiation generation device individually according to the current moving state of the target object and the contents of the lookup table. As the system uses wireless transmission, and the lookup table has recorded the working state of each radiation emitter in each moving state of the target object, radiotherapy can be performed without a large number of tubes or sensors.

The techniques include operations comprising: obtaining, during delivery of a radiotherapy treatment fraction to a patient, one or more images of the patient at a first rate; generating patient motion information at a second rate based on the one or more images obtained at the first rate; receiving, during delivery of the radiotherapy treatment fraction, radiotherapy treatment device settings at a third rate; computing, during delivery of the radiotherapy treatment fraction, dose delivered to the patient with a first level of accuracy based on the generated patient motion information and the radiotherapy treatment device settings; and determining, during delivery of the radiotherapy treatment fraction, a real-time measure of accumulated dose delivered to the patient with a second greater level of accuracy than the first level of accuracy using one or more prior dose computations.

The method includes generating a plurality of projections of an internal target region within a body of a patient, the plurality of projections comprising projection data about a position of an internal target region of the patient. The method further includes generating external positional data about external motion of the body of the patient using one or more external sensors. The method further includes generating, by a processing device, a correlation model between the projection data and the external positional data by fitting the plurality of projections of the internal target region to the external positional data. The method further includes estimating the position of the internal target region at a later time using the correlation model.

Systems and methods are disclosed for monitoring anatomic position of a human subject for a radiotherapy treatment session, based on use of a regression model trained to estimate movement of a region of interest based on 2D image data input. Example operations for movement estimation include: obtaining 3D image data for a subject, which provides a reference volume and at least one defined region of interest; obtaining 2D image data corresponding to the subject, captured in real time (during the radiotherapy treatment session); extracting features from the 2D image data; analyzing the extracted features with a machine learning regression model, trained to estimate a spatial transformation in the three dimensions of the reference volume; and outputting and using a relative motion estimation of the at least one region of interest, produced from the machine learning regression model, the relative motion estimation being estimated from the extracted features.
