Abstract
A device (24) for placing a seal around an implant in a blood circulation passage is provided. The device includes a hollow sheath having a longitudinal axis, and at least one releaser, deployable relative to the sheath is provided. The device (24) includes, for the or each releaser, a sealing body (64) releasably fastened on that releaser, the sealing body (64) having an angular span smaller than 360° around the longitudinal axis, and a filiform link (67) connecting at least one releaser to another releaser and/or to the controller (68) positioned in the sheath, the filiform link (67) being deformable between a relaxed idle configuration and a stretched configuration limiting the radial movement of the sealing bodies (64).
Claims
1. A device for placing a seal around an implant in a blood circulation passage comprising: a hollow sheath having a longitudinal axis; one or more releasers deployable relative to the sheath between a position retracted in the sheath and a position deployed outside the sheath; for the or each releaser, a sealing body releasably fastened on the corresponding releaser to be released from the releaser, the sealing body having an angular span strictly smaller than 360° around a longitudinal axis; and at least one filiform link connecting at least one of the releasers to another releaser and/or to a controller positioned in the hollow sheath, the filiform link being deformable between a relaxed idle configuration and a stretched configuration limiting the radial movement away from the longitudinal axis of the sealing bodies, the or each sealing body and the or each filiform link being movable in the blood circulation passage independently from the implant.
2. The device as recited in claim 1 wherein the at least one releaser is a plurality of releasers spaced angularly apart from one another around a longitudinal axis, each releaser bearing one of the sealing bodies releasably fastened on the releaser to be released from the releaser, the at least one filiform link connecting one of the releasers to an adjacent one of the releasers.

The device as recited in claim 2 wherein the at least one releaser is at least three releasers, the at least one filiform link being a plurality of filiform links, each of the filiform links connecting each releaser to each of the adjacent releasers, the filiform links forming a peripheral chain delimiting a central space.

The device as recited in claim 1 wherein the at least one filiform link connects one of the releasers to at least one controller, the or each controller being inserted into the sheath independently of the or each releaser, the or each controller having a proximal end able to be manipulated by a practitioner and a distal end on which the at least one filiform link is engaged.

The device as recited in claim 4 wherein the or each controller comprises at least one maneuvering stay able to be rotated from a proximal end of the stay, the at least one filiform link being able to be wound on a distal end of the stay during the rotation to come transversely closer to the sealing body corresponding to the longitudinal axis.

The device as recited in claim 5 wherein the one or more releasers is a plurality of releasers spaced angularly apart from one another around a longitudinal axis, each releaser bearing one of the sealing bodies releasably fastened on the releaser to be released from the releaser, the at least one maneuvering stay being at least two maneuvering stays, each maneuvering stay being able to wind one of the filiform links connecting the maneuvering stay to one of the releasers.
7. The device as recited in claim 6 wherein the stays are mounted telescoping in one another.
8. The device as recited in claim 4 wherein the or each controller has a loop engaged on the at least one filiform link.
9. The device as recited in claim 8 wherein the one or more releasers is a plurality of releasers spaced angularly apart from one another around the longitudinal axis, each releaser supporting one of the sealing bodies releasably fastened on the releaser in order to be released from the releaser, the filiform link on which the loop is engaged connecting two of the releasers.
10. The device as recited in claim 4 wherein the at least one filiform link is releasably connected to the at least one controller.
11. The device as recited in claim 1 wherein the or each filiform link is formed from a silicone thread.
12. The device as recited in claim 1 wherein the or each sealing body comprises a radiopaque marker.
13. The device as recited in claim 1 further comprising a filter deployable transversely around the sheath.
14. A treatment kit for a blood circulation passage comprising: an implant designed to be implanted in a blood circulation passage and comprising an endoprosthesis deployable between a contracted state and an expanded state, the endoprosthesis advantageously bearing a valve; the device as recited in claim 1, the or each sealing body being designed to be positioned around the implant in the blood circulation passage.
15. The treatment kit as recited in claim 14 further comprising a release tool for releasing the implant able to be inserted into the blood circulation passage independently of the placement device.
16. The treatment kit as recited in claim 14 wherein the endoprosthesis bears a valve.

(19) The endoprosthesis 30 has a tubular shape with axis X-X′. It delimits a central blood circulation passage 32 emerging axially on either side of the endoprosthesis 30. The endoprosthesis 30 bears a valve 34 closer attached on the endoprosthesis 30 inside the passage 32.

The framework 36 is obtained by braiding at least one thread made from stainless steel, a shape memory alloy, or a polymer. (22) The inner surface 38 inwardly defines the central passage 32. The outer surface 40 is designed to be pressed at least partially against the wall 16 and/or against the leaflets 18, as will be seen below.

(25) Traditionally, the valve 34 comprises a tubular base 46 fastened on the inner surface 38 of the framework 36, and several flexible leaflets 48 closing off the central passage 32 that extend the base 46 downward.
(26) The leaflets 48 are movable radially toward the axis X-X′ of the passage 38 between a closing position, in which they substantially completely prevent blood from passing through the passage 32, and a position freeing the passage 32, in which they are substantially pressed against the inner surface 38 and allow blood to pass through the passage 32.

(107) The filiform links 67 follow the releasers 62 in their radial expansion. The central controllers 68 in turn follow the associated filiform links 67.
(108) During that transition, each sealing body 64 comes closer to the wall 16 delimiting the passage 14 to advantageously come into contact with that wall 16.
(109) If the implantation point is situated in a native valve 12, the practitioner for example inserts each sealing body 64 into a commissure 20 of the valve 12 situated between two adjacent native leaflets 18.
(110) Then, the practitioner moves the closer 69 toward the distal end 72 to reach the intermediate separated position. The nose 112 then cooperates with each releaser 62 to cause an additional radial separation of the sealing body 64 fastened on the releaser 62.
(111) Then, the practitioner specifies the position of each sealing body 64 in the corresponding commissure 20 by pulling the proximal end 81 of the central controller 68 corresponding to that sealing body 64.
(112) This action causes the deformation of the corresponding filiform link 67 from its relaxed idle position to its stretched position. This thereby limits the radial separation of the pair of sealing bodies 64 connected to the filiform links 67.
(113) It is thus possible to partially retract, toward the axis Y-Y′, each sealing body 64 to adjust its position relative to the commissures, by actuating an associated central controller 68, causing proximal traction on the filiform links 67 and associated radial movement of the releaser 62.
(114) This next makes it possible to pivot the releasers 62 around the axis Y-Y′, based on the patient's morphology, to ensure that the sealing body 64 is indeed placed in the desired commissure.
(115) Then, the practitioner actuates the deployment of the filter 79 to recover any calcification debris released from the native leaflets 18.
(116) The central space 90 situated between the releasers 62 and the central space 103 delimited by the filiform links are freed. The practitioner next inserts an additional surgical guide up to the central space 90, advantageously by a path opposite the insertion path of the guide 70.
(117) The practitioner then guides the release tool of the implant 22 onto the additional guide and deploys the implant 22 into the central space 90, across from each sealing body 64, as illustrated by the

(119) Each sealing body 64 and each filiform link 67 are therefore interposed between the implant 22 and the peripheral wall 16 in the blood circulation passage 14, to substantially plug the intermediate space present between the implant 22 and the wall 16, in particular at the commissures 20 of the native valve 12.
(120) Then, the practitioner releases each sealing body 64 from the releaser 62 on which it is fastened. To that end, it moves the retainer 96 from its configuration engaged with the sealing body 64 to the release configuration, while keeping the sealing body 64 substantially in position by bearing on the releaser 62.
(121) The practitioner further releases each filiform link 67 of the corresponding central controller 68.
(122) Then, the practitioner actuates the contraction means to contract the filter 79 in its initial position.
(123) The practitioner next retracts each releaser 62 into the sheath 60, then withdraws the sheath 60.
(124) The device 24 allows positioning of each sealing body 64 very precisely and in a suitable manner with a variable structure of a native valve.
(125) Furthermore, the radiopaque markers contained in the sealing bodies 64 allow the practitioner to see the position of each sealing body 64 and to correct it if needed.

(135) The operation of the kit 130 according to the second embodiment of the invention differs from the operation of the kit 10 according to the first embodiment in that, in order to specify the position of each sealing body 64, the practitioner pivots the proximal end 170 of the corresponding central controller 168 to wind the filiform link on its distal end. This then makes it possible to bring the corresponding sealing body closer to the longitudinal axis Y-Y′.
