The present invention relates to novel equipment's and methods for minimally invasive surgery emphasizing use of rigid platform for transoral endoscopic surgery of upper gastrointestinal tract. The present novel equipments and methods using the rigid platform aids in effective surgical procedures through natural orifice with accuracy, minimum bleeding, and under vision scarless major surgery of upper gastrointestinal (UGI) tract such as transoral cardiomytomy for achalasia cardia (TOEM), transoral fundoplication (TOF) for hiatus hernia, transoral bariatric surgery (TOBS), transoral oesophagectomy (TOO) for oesophageal cancers and NOTES UGI surgery. Present invention provides a Transoral Port (TOP) used for performing natural orifice surgery of the upper gastrointestinal tract; which mainly comprises of Sheath (SH), Blunt Introducer (BI), Telescope (TE), Diaphragm (DI), Silicon washers (SW), Tunnel pusher tube (TP), Airport (AP), Suction port (SP), Bevel (AB or PB), Markings (MA), Monitor (MO).

A cannula seal is presented. The cannula seal includes a cross-slit seal that can include a side wall that includes folded sidewalls; slits that are formed by the folded sidewalls when the cross-slit seal is in a closed position; and a concave end surface at the slits. The cross-slit seal can include energizing ribs formed to close the seal. The instrument guide includes a pyramidal shaped seal that seals against doors when no instrument is inserted and seals against an instrument when the instrument is inserted.

A surgical device, an outer tube, an endoscope, and a treatment tool that have a compact configuration and achieve high durability are provided. The outer tube includes a slider in an outer tube body. When the endoscope and the treatment tool are inserted into the outer tube, and are held by an endoscope holding part and a treatment tool holding part each being provided in the slider, and are combined into a single unit. Consequently, when the treatment tool is moved in the axial direction, the endoscope is moved in the axial direction in interlock with the movement. Because the outer tube is configured such that the endoscope exit port has an inner diameter that is smaller than the inner diameter of the elastic member provided to hold the endoscope at the endoscope holding part, the strength of the endoscope can be secured while preventing interference with the treatment tool.

The inner hub can include a cannula port. In some embodiments, the assistant port device can be utilized for skin retraction during single port teleoperated robotic surgery.

Methods and devices for providing access into a body cavity

Methods and devices are provided for providing surgical access into a body cavity. In one embodiment, a surgical access device is provided that includes a housing coupled to a retractor. The housing can be have one or more movable sealing ports for receiving surgical instruments. Each movable sealing port can include one or more sealing elements therein for sealing the port and/or forming a seal around a surgical instrument disposed therethrough. Each movable sealing port can be rotatable relative to the housing and each sealing element can be rotatable relative to the housing along a predetermined orbital path.

Apparatus for providing instrument access through a surgical access device

An apparatus for providing instrument access through a gel pad of a surgical access device disposed to seal an opening to a body cavity of a patient is disclosed. The apparatus includes a stem having a bore extending therethrough, the stem having a length sufficient such that when the stem is inserted through the gel pad a distal end of the stem protrudes beyond an inner surface of the gel pad. The bore is sized to permit an instrument to be inserted through the bore and through the opening into the body cavity. The stem includes a flanged opening to the bore that remains external to an outer surface of the gel pad and limits the insertion of the stem through the gel pad. The apparatus also includes a displaceable seal disposed within the bore below the flanged opening, the displaceable seal being operable to seal the bore prior to insertion of the instrument and to displace to permit insertion of the instrument while maintaining the seal.
