An implantable medical device (IMD) includes therapy delivery circuitry, sensing circuitry, and processing circuitry. The processing circuitry is configured to determine one or more sleep apnea therapy parameters, control the therapy delivery circuitry to deliver sleep apnea therapy via a first set of electrodes implantable within the patient in accordance with the one or more sleep apnea therapy parameters, and at least one of: (1) monitor a cardiac signal sensed with the sensing circuitry, or (2) determine one or more cardiac therapy parameters, and control the therapy delivery circuitry to deliver cardiac therapy via a second set of electrodes implantable within the patient in accordance with the one or more cardiac therapy parameters.

Methods and systems for controlling blood pressure by controlling atrial pressure

Systems and methods for controlling blood pressure by controlling atrial pressure and atrial stretch are disclosed. In some embodiments, a stimulation circuit may be configured to deliver a stimulation pulse to at least one cardiac chamber of a heart of a patient, and at least one controller may be configured to execute delivery of one or more stimulation patterns of stimulation pulses to the at least one cardiac chamber, wherein at least one of the stimulation pulses stimulates the heart such that an atrial pressure resulting from atrial contraction of an atrium overlaps in time a passive pressure build-up of the atrium, such that an atrial pressure of the atrium resulting from the stimulation is a combination of the atrial pressure resulting from atrial contraction and the passive pressure build-up and is higher than an atrial pressure of the atrium would be without the stimulation, and such that the blood pressure of the patient is reduced.

Systems and methods for generating therapeutic electric fields for capture of nervous system targets

A neuromodulation system includes neuromodulation electrodes on an electrode support that is positionable within a blood vessel for transvascular stimulation of target nerves. Cathode surface area, cathode-to-anode spacing, and other parameters are selected for capture of specific types of nerves (sympathetic, parasympathetic, and/or mixed nerves), nerves located at larger or shorter distances from the vascular wall, and also for proper nerve capture for the targeted types of nerves.

Electrode assemblies for neurostimulation treatment

A device includes a handle, an expandable structure including a plurality of splines extending from a proximal hub to a distal hub, a first electrode on a first spline of the plurality of splines, an outer tube extending from the handle to the proximal hub, and a shaft extending through the outer tube from the handle to the distal hub. The expandable structure has a collapsed state and a self-expanded state. The handle is configured to retract the shaft. Retracting the shaft may expand the expandable structure outward of the self-expanded state.

Methods and apparatus to stimulate the heart
11759639 · 2023-09-19 · ·

A method and apparatus for treatment of hypertension and heart failure by increasing secretion of endogenous atrial hormones by pacing of the heart. Pacing is done during the ventricular refractory period resulting in premature atrial contraction that does not result in ventricular contraction. Pacing results in the atrial wall stress, peripheral vasodilation, ANP secretion. Concomitant reduction of the heart rate is monitored and controlled as needed with backup pacing.

Implantable assembly

An implantable assembly is described for acquisition of neuronal electrical signals at a selected location which propagate along at least one nerve fiber contained in a nerve fiber bundle, as well as for selective electrical stimulation of the at least one nerve fiber, having: an implantable electrode assembly (E) which is disposed on a biocompatible support substrate which can be positioned around the nerve fiber bundle in a cuff. The cuff has a cylindrical support substrate surface (i) which in the implanted condition is orientated facing the nerve fiber bundle, on which a first electrode assembly for locationally selective acquisition of the neuronal electrical signals and selective electrical stimulation of the at least one nerve fiber, and on which a second electrode assembly is disposed to record an ECG signal, and an analysis and control unit (A/S) which can be electrically conductively connected or is connected to the implantable electrode assembly (E), in which the locationally selective acquired neuronal electrical signals as well as the ECG signal can be analyzed in a time-resolved manner such that a neuronal time signal correlated with a physiological parameter, such as blood pressure, can be derived.
