In some examples, the medical device may be an implantable medical device that is coupled to an extra-cardiovascular lead for delivering the cardiac pacing pulses.

Methods and systems for regulating nerve activity and/or treating conditions associated with disorder of blood glucose are disclosed. A method of downregulating activity by applying a high frequency alternating current electrical signal to a nerve in a subject is disclosed. A method of upregulating activity by applying a low frequency stimulation signal to a nerve in a subject is disclosed. A method of regulating nerve activity by applying a high frequency signal to a first nerve/organ and applying a low frequency stimulation signal to a second nerve/organ is disclosed. The application of the high frequency signal and the low frequency stimulation signal to separate nerves or nerve branches/fibers can be independent, simultaneous, concurrent, or in a coordinated fashion in therapy programs. Various signal parameters including the waveform, frequency, amplitude, active/inactive phases are described.

An implant system includes an electrode portion comprising plural electrodes to perform nerve stimulation and nerve sensing, an impedance controller configured to selectively connect the plural electrodes between a stimulator to perform nerve stimulation and a sensor to perform nerve sensing based on a control signal, and set an impedance of each of the plural electrodes, and a processor configured to control a contact impedance by the plural electrodes by generating the control signal to control at least one of plural switches connected respectively to the plural electrodes, or variable resistors connected respectively to the plural electrodes, based on at least one of a selectively set purpose of the plural electrodes or a position of an electrode to which nerve stimulation is to be provided.

Restless Leg Syndrome (RLS) or Periodic Limb Movement Disorder (PLMD) can be treated using high frequency (HF) electrostimulation. This can include selecting or receiving a subject presenting with RLS or PLMD. At least one electrostimulation electrode can be located at a location associated with at least one of, or at least one branch of, a sural nerve, a peroneal nerve, or a femoral nerve. HF electrostimulation can be delivered to the subject, which can include delivering subsensory, subthreshold, AC electrostimulation at a frequency that exceeds 500 Hz and is less than 15,000 Hz to the location to help reduce or alleviate the one or more symptoms associated with RLS or PLMD. A charge-balanced controlled-current HF electrostimulation waveform can be used.

A method of applying a neural stimulus with an implanted electrode array involves applying a sequence of stimuli configured to yield a therapeutic effect while suppressing psychophysical side effects. The stimuli sequence is configured such that a first stimulus recruits a portion of the fibre population, and a second stimulus is delivered within the refractory period following the first stimulus and the second stimulus being configured to recruit a further portion of the fibre population. Using an electrode array and suitable relative timing of the stimuli, ascending or descending volleys of evoked responses can be selectively synchronised or desynchronised to give directional control over responses evoked.

Methods for treating a subject for a parasympathetic bias mediated condition are provided. Aspects of the methods include modulating at least a portion of the subject's autonomic nervous system to increase the sympathetic/parasympathetic activity ratio in a manner effective to treat the subject for the parasympathetic bias mediated condition. In some instances, the subject is known to have parasympathetic bias. Also provided are devices that find use in practicing various embodiments of the methods.

The pulses of the electrical stimulation signal may each have a relatively low amplitude, but due at least in part to a relatively high number of pulses per unit of time, a dose of the electrical stimulation may be high enough to elicit a therapeutic response from the patient.
