In certain embodiments, a vial adaptor for removing liquid contents from a vial comprises a piercing member and a bag. The bag can be contained within the piercing member such that the bag is introduced to the vial when the vial adaptor is coupled with the vial. In some embodiments, the bag expands within the vial as liquid is removed from the vial via the adaptor, thereby regulating pressure within the vial. In other embodiments, a vial comprises a bag for regulating pressure within the vial as liquid is removed therefrom. In some embodiments, a vial adaptor is coupled with the vial in order to remove the liquid. In some embodiments, as the liquid is removed from the vial via the adaptor, the bag expands within the vial, and in other embodiments, the bag contracts within the vial.

The first and second piercing members may be isolated from the flow channel and may respectively puncture the first and second cover in the spiked position.

A removable closure assembly has a cap making a liquid-tight seal with a tri-clamp assembly to the open end of a filter assembly body. The closure assembly has a filter which fits within the filter body. To aid in separating the closure assembly from the body, the cap has an outwardly extending shaft fixed thereto. The shaft is terminated by a radially protruding stop block. A handle is slidable on the shaft and functions as a slide hammer weight with an end wall facing the cap through which the shaft extends. The handle is movable to strike the shaft stop block and apply a force to the stop block and shaft for separating the cap from the body. The stop block has axial channels allowing a clean out of place system to exchange cleaning fluid with an interior volume of the handle.

The reservoir can be contained within the piercing member and the insertion member, such that the reservoir is introduced to the vial when piercing member is inserted into the vial. The piercing member is adapted to be opened inside the vial to expose the reservoir to the contents inside the vial. A locking mechanism can prevent the piercing member from being inserted into the vial unless the vial access device is fully coupled to the vial and/or prevent the piercing member from being withdrawn from the vial without uncoupling the vial access device from the vial.
