Implementations of an inkjet based roll with punch-out cards comprising a carrier sheet having a top side and a bottom side, where the carrier sheet comprises a plurality of cut-outs, where for each cut-out, a patch/tape is applied to the bottom-side of the carrier sheet to cover the cut-out, and a card (e.g., a polyester plastic card) is attached to the patch through the cut-out from the top side of the carrier sheet. In some implementations, the card and the patch comprise perforations to create a punch-out card. In some implementations, the patch/tape is made from an opaque material having an adhesive on a first side on which the card is adhered. In some implementations, a method of using the inkjet based roll with punch-out cards comprises cutting a plurality of holes in a roll of a carrier sheet where the carrier sheet has a top side and a bottom side. In some implementations, the method further comprises applying to the bottom-side of the carrier sheet a patch/tape to cover the cut-out. In some implementations, the method further comprises attaching a card (e.g., a polyester plastic card) to the patch through the cut-out from the top side of the carrier sheet. In some implementations, the method further comprises perforating the patch and the card from the bottom-side of the carrier sheet. In the method, in some implementations, UV light is not used to dry the adhesive of the patch/tape.

Described herein is form assembly having a removable and synthetic peel out identification card and a method of making the same. The assembly is generally comprised of a laminate of a first planar form layer having an at least one aperture, a first planar layer of synthetic material, and a second planar layer of synthetic material forming the identification card. The first planar layer of synthetic material is applied to a second side of the first planar form layer across the width and over the at least one aperture forming a backer. The second planar layer of the synthetic material is placed within the at least one aperture and secured to the first planar layer of synthetic material. A controlled depth cut is then made to a second side of the first planar layer of synthetic material within a perimeter of the second planar layer of synthetic material to allow for the selective release of the second planar layer of synthetic material from the form.

A gift card folder for securing a gift card is described. The gift card folder may be composed of a single sheet of substantially rectangular stationary which has a plurality of creases dividing the single sheet of stationary into panels of which two panels cooperate together to form a pop-up shelf upon manipulation of the gift card folder. A portion of the pop-up shelf comprises one or more slits to secure the gift card in a fixed position during opening or closing of the gift card folder. Further, the gift card folder may optionally include a flap that cooperates with an outer panel to secure the gift card folder in a closed position which compresses the pop-up shelf. When the recipient opens the folder, the gift card folder projects the pop-up shelf outward and towards the recipient of the gift card.

The card can be a gift card, a customer loyalty card, or a customer incentive card. An interface may be connected to a communications network that is accessible to associate the identifier with an account and manage funds associable with the account. Funds data may be associable with the account and storable on the remote server, the funds data relating to the funds transferred by a user to a card issuer. The identifier may be storable on the card in an electronically readable format, and may be included on the card via printing and/or imprinting. A method is provided for using the numbering system.

Printable sticker form with a manual adhesive transfer/placement system

Forms that can be used to make stickers easily on demand, wherein the stickers are of the type in which a face of the sticker that bears printed information (graphic, textual, bar code, security, and/or other) also bears an adhesive that is used to affix the sticker to a desired substrate. A carrier sheet supports a sticker body and an adhesive source. The sticker body and the adhesive are initially separated on the form. The sticker face and the adhesive are registrably positioned with respect to a fold line. This allows the adhesive to be transferred to the sticker face on demand by an easy, desirably manual process that involves folding the form along the fold line. When the form is unfolded, the sticker can be removed and applied to the desired substrate.
