Disclosed is a fenestrated bone anchor suitable for being anchored in live bone tissue of a human or animal patient. The bone anchor comprises a shaft with a proximal end and a distal end and a circumferential surface extending from the proximal end to the distal end. The shaft further comprises a longitudinal cavity extending from the proximal end towards the distal end and a plurality of lateral channels extending through a wall of the shaft from the axial cavity to the circumferential surface. The lateral channel has a pear-shaped cross section and/or the thickness of the wall increases gradually in selected ones of directions towards the lateral channels. The fenestrated bone anchor is e.g. a component of a surgical system comprising an interbody fusion device of the stand-alone type.

An anchor for anchoring tensile members to bone includes: a housing extending along a central axis, with a hollow interior; a collet in the hollow interior having a central bore for accepting tensile members and an exterior surface, the collet being configured to swage around and against tensile members; a sleeve having a peripheral wall defining interior and exterior surfaces, the sleeve disposed in the housing's hollow interior axially adjacent to the collet, and movable parallel to the central axis between first and second positions; and wherein at least one of the collet exterior surface and the sleeve interior surface is tapered and the sleeve and the collet are arranged so movement of the sleeve from the first position to the second position causes the sleeve interior surface to bear against the collet exterior surface, causing the collet to swage radially inwards around and against one or more tensile members.

The implant includes a trunk, a distal anchor, a proximal anchor, and a stem coupled to the trunk. A driver of the insertion tool receives an input for a drive shaft of the insertion tool to draw the stem proximally such that the trunk is drawn towards the insertion tool. The drawing of the trunk and engagement of first and second engagement features is configured to deploy the proximal anchor outwardly. The distal anchor may be an expandable member, and the implant may define a bore extending through the proximal anchor for the distal anchor to receive injectable material. The system may be cannulated such that the insertion tool and the implant may be directed over a guidewire to position the implant.

The shaft includes blades disposed within the central portion, the blades movable between a closed position in which the blades are disposed within the shaft and an open position in which the blades extend laterally through the shaft. The post includes a threaded outer surface threadingly engaged with the threaded inner surface of the shaft. The post is movable longitudinally within the shaft to transition the blades between the closed and open positions.
